The Confusion Assessment Method is a commonly used and validated tool for detecting confusion in patients with dementia or delirium. It assesses four features of confusion: acute onset or fluctuating course, inattention, disorganized thinking, and altered level of consciousness.
The Geriatric Depression Scale assesses depression in older adults, while the Mini-Cog and Clock Drawing Test are screening tools for dementia and cognitive impairment. While these tools may be useful in a comprehensive assessment of cognitive status, the CAM is specifically designed to screen for confusion in patients with dementia or delirium.

Presentations of hyponatremia include the following:
* Apprehension
* Seizures
* Coma
* Hypotension
* Tachycardia
* Decreased urine output
* Weight gain
* Edema
* Ascites
* Jugular vein distension
Presentations of hypernatremia include:
* Convulsions
* Pulmonary edema
* Thirst
* Fever
* Dry mucous membranes
* Hypotension
* Tachycardia
* Low jugular venous pressure
* Restlessness

The PMH-BC should rule out IPV. Although the patient is a diabetic and some, if not all, of her symptoms can be explained by a lack of self-care, they are also signs of IPV.
The husband's refusal to leave the patient alone and insistence on answering her questions are suspicious. Victims of IPV often look unkempt and may present with signs of malnourishment and dehydration. Missed medical appointments and lack of self-care are also signature signs.

* Home health aides
* Skill nursing facilities services for those age 21 and older
* Services rendered by nurse practitioners and nurse midwives

* Eyeglasses
* Dental care
* Dentures
* Hearing exams
* Hearing aids
* Routine physical exams
* Inpatient psychiatric hospitalizations
